We are seeking Palliative Care Nurse Practitioners (NP) who will play an integral part in supporting the goals of the Integrated Palliative Home Care Program and Regional Steering Committee to ensure high quality hospice palliative care across various health care settings in the Mississauga Halton catchment area.

The goals of the program are to (i) Enhance continuity of clinical care coordination across primary care, home care, community supports, acute and speciality palliative care sectors; (ii) Reduce hospitalization/avoidable emergency department visits for palliative clients; (iii) Enhance quality of palliative care by pain and symptom management by supporting individuals in dying in their place of choice (including home) through NP’s advanced scope of practice; (iv) Build CCAC foundational capacity for 24/7 palliative care; and (v)Strengthen capacity of primary care practitioners providing palliative care.

Team: Palliative Care Nurse Practitioners

Primary assigned location: 2655 North Sheridan Way, Mississauga                                 

POSITION OUTLINE:

Nurse Practitioners use highly developed consultation skills to coach and mentor inter-professional team members working collaboratively across the health care system, including home care, primary care, specialized palliative care, acute care and community services. NPs will provide expert clinical palliative leadership to support seamless, integrated care delivery. In addition to clinical practice the NPs will engage in all domains of advanced practice nursing (APN) including leadership, collaboration and research.

Key accountabilities of this position include:

  • Support Most Responsible Physician (MRP) to ensure continuity of medical/clinical care for individuals with complex palliative needs across primary, acute, community and specialized palliative care sectors in relation to coordinated management approaches, information exchange and relationships with the client and family
  • Provide clinical consultation in the assessment, planning, implementation and evaluation of patient care
  • Identify complex needs that may require tertiary intervention
  • Collaborate with primary level of care providers, disease specialists or tertiary providers
  • Identifies researchable palliative care practice questions and links with appropriate academic facilities and resources
  • Participates in the identification of the educational needs of the inter-professional team and facilitates and participates in the planning and delivery of education to meet those needs
  • Provides mentorship and role models critical thinking, problem solving and the use of evidence to guide decision making amongst the health care team and help guide system design
  • Participate in shared care roles where client needs are complex and the primary care team will benefit from ongoing consultation and recommendations from an APN
  • May provide secondary level on call support to primary level of care providers for the purposes of assistance with problem solving and decision-making
  • Participate in rounds with geographically-based palliative care teams to identify complex palliative needs and assist in care planning in an inter-professional setting

Qualifications include:

  • Current registration with the College of Nurses of Ontario in the Extended Class (NP-Adult or NP-Primary Health Care). RNs without the extended class designation may be considered provided they are currently enrolled in the NP program and commit to obtaining this designation within three (3) years.
  • CHPCN(C) certification an asset
  • Masters level preparation in clinical nursing preferred
  • Continuing education in palliative care
  • Minimum of three (3) years’ experience in Palliative Care Nursing, preferably in a community setting
  • Effective interpersonal, organizational and planning skills
  • Able to communicate with clients’, their families, and other relevant individuals in order to follow through with care plan directives
  • Demonstrated experience with proven team building abilities and experience in advancing the clinical practice of multiple health disciplines
  • Demonstrated advanced knowledge in consultation and ethical decision-making
  • Demonstrated use of theory and evidence to advance clinical practice and outcomes
  • Demonstrated awareness of cultural diversity, as well as ability to behave discreetly and sensitively to confidential issues
  • Proficiency with computerized information systems
  • Valid driver’s license and access to a motor vehicle
  • Insurance that includes driving for business purposes and minimum liability of$1,000,000.
  • Ability to communicate in French or other languages an asset.
