When is the right time to transition? What steps can be taken to ensure that the transition process is a smooth one for the affected child and family? These are some of the important questions that will be tackled by ‘guest-tweeters’ in the International Children’s Palliative Care Networks(ICPCN) upcoming #CPCDialog tweetchat entitled ‘transition from child to adult palliative care services.’
Guests for this tweetchat will include Lizzie Chambers and Lucy Watts MBE. Lizzie Chambers leads the Together for Short Lives Transition Taskforce, which is a strategy group involved in helping to improve the experiences of young people with life-limiting conditions as they move into adult services. Lucy Watts MBE is ICPCN’s Global Youth Ambassador, as well as an ambassador for Together for Short Lives. Lucy is an outspoken advocate for the rights of young people living with life-limiting conditions to access the services that are necessary for their needs and age group.
