Salary: £27,343 – £36,525 p.a. pro rata (depending on experience)

Equivalent to AfC Band 6        

Hours: Full Time/Part Time (minimum 30 hours)

Princess Alice Hospice is one of the top ten adult hospices in the UK providing specialist palliative care and support to a community of more than a million people living in Surrey, south West London and Middlesex. Our superb Hospice building gives home to our Day Services and a purpose-built In Patient Unit with the capacity to care for 24 highly complex patients at any one time, offering those who need to stay with us a private, en-suite room with large doors that open out onto our garden, courtyard or decking area.

An exciting opportunity has arisen for an experienced healthcare professional (i.e. nurse, physio, occupational therapist) to join our team and work alongside members of the Hospice Multi-Disciplinary Team as a Discharge Coordinator/Family Support Liaison. Based principally in the Specialist Palliative Care In-Patient Unit, you will work towards ensuring a smooth transition for patients between the In-patient unit and community/other settings on discharge, and to support patient and families throughout their stay within the In-Patient Unit.

We are looking for a motivated individual with strong organisational, negotiation and communication skills, a strong sense of team spirit and a passion for delivering high quality, compassionate patient and family focused care.

You should be a registered healthcare professional with significant relevant experience, ideally some of which within a palliative care or community setting. Knowledge of Health and Care needs assessment processes and documentation would be an advantage, however this is not essential as training will be given.
