Salary: £20,040 per annum

Hours of work: 37.5 hours per week to be worked over 5 days at Warwick Myton Hospice to meet the needs of the service

The Myton Hospices are committed to the delivery of high quality specialist palliative care and our service aims to ensure patients with palliative and end of life care needs are able to be cared for in their preferred place of choice. We are pleased to announce that we have an exciting vacancy that has become available for a full time Discharge Liaison Co-ordinator based at our 20 bedded Hospice in Warwick.

The Discharge Liaison Co-ordinator will work closely with our Multi Disciplinary Team and provide specialist discharge advice/support to patients, family and other health care professionals.

Applicants should be either a Registered Nurse or Allied Healthcare Professional with a relevant degree or working towards with significant post registration experience in Cancer/Palliative Care.

Free onsite car parking is available at all Myton Hospice sites.

If you are already a member of the NHS Pension Scheme when you join The Myton Hospices as a Discharge Liaison Co-ordinator you will be able to continue with your NHS Pension.
