Palliative Care Social Worker – working with adults and families

Circa £27K p.a. (part time 22.5 hours pro-rata)

Due to exciting service expansion at St Clare, we are looking for new members to join our team, helping us lead the way for local Hospice care.

Patients and families are at the heart of all we do. We pride ourselves on having a team who can deliver excellent care, of exceptional quality all tailored to the individuals we care for.

Are you a qualified, motivated experienced Social Worker able to work with families, adults and children, wishing to be part of an exciting new team working in Loss and Transitions?

We are looking for an energetic, creative person to work as a member of the Hospice multi-disciplinary team providing a holistic approach to patients and their families.
