Specialist Practitioner for the Palliative Care Coordination Centre
Salary range: £35,100 – £37,800 per annum
Full time, 37.5 hours per week working a shift pattern over seven days which will include OOH, weekends and bank holidays

St Catherine’s Hospice provides inpatient, day care and community specialist palliative and supportive care to people with life limiting illnesses and their families within Sussex and Surrey.
We are seeking an innovative and motivated individual, who ideally has excellent assessment skills, case management experience and palliative care experience to join our team as a specialist practitioner for our new Palliative Care Co-ordination Centre. This service will provide a single point of contact for patients, families and health and social professionals as well as being the main access point for all new referrals to the hospice, as well as providing round the clock advice and support for patients, families and health and social care professionals.
You should have sound leadership and management experience, as well as excellent communication and interpersonal skills. You should also be able to work using your own initiative, have excellent prioritisation and assessment skills, be able to give clear instruction and lead and manage a team of practitioners and administrators.
