Supportive Outpatients – Team Leader

Salary: £23,935 – £33,909 per annum, pro rata
Working 30 or 37.5 hours, over 4 or 5 days

This is an exciting opportunity for a health or social care professional who is motivated, committed and dynamic and can lead and develop the Supportive Outpatient Service at the Neil Cliffe Centre.

Supportive Outpatients provides a range of services for patients and their carers with a diagnosis of life limiting disease, at any stage of their illness.

The service is currently under review so we are seeking an individual who can be instrumental in leading the implementation of the service developments in collaboration with the Clinical Operational Manager. 

The service currently runs 5 days a week, with a weekly medical outpatient clinic and various outpatient services.

The role involves leading a team of key workers and other clinical staff, administration staff, support services and volunteers to meet the person’s supportive and palliative care needs.

You will be a source of expert clinical advice and have direct clinical involvement by managing your own clinical caseload.

You will need to be an organised individual with excellent communication and leadership skills.
