Salary: £20,705 pro rata per annum
Hours: 22.5 hours per week (with flexibility for some weekends)

ellenor are the provider of all-encompassing hospice care for both adults and children living in Dartford, Gravesham and Swanley, West Kent and Bexley. Our services include adult In-patient and Day Therapy at the Hospice in Northfleet, and 24 hour Hospice at Home for all ages. Our strategic aims recognise our role in leading the development of services to the wider community at the end of life, and we see an increasing role in education and training and as a coordinator for providing the best possible palliative care to more people and families.

We are currently recruiting for a Transition Worker to join our Children’s Hospice Care team.

The children and young people we work with have life limiting or life threatening conditions and their needs vary greatly. You will be required to demonstrate an excellent understanding of the particular issues facing young people and families at the point of transition, as well as being able to work effectively with young people, their families and wider professional networks to effectively assess, plan and co-ordinate the delivery of services to ensure smooth transition into adulthood.

This post is subject to Enhanced DBS clearance. Support with professional development will be provided. An employer contributory pension scheme is available, and NHS Pension Scheme members can transfer in their pension.
