This study day will be of interest to nurses, therapists, doctors and other healthcare practitioners who wish to develop confidence, skills and understanding about the processes and conversations around Advance Care Planning.
Advance Care Planning (ACP) is an ongoing process about preferences, choices of care and future care needs between a patient and a healthcare professional. It may/may not include family or friends.
The day will include an appreciation of what it is and isn’t, considering other end of life care planning, within the context of current research.
This interactive day will be about providing opportunities to understand the nitty-gritty of Advance Care Planning. We will be exploring how to pick up on cues from patients, providing practical tips and hints to aid practice within a safe environment
This study day will help participants to:
- Develop an understanding of the processes of Advance Care Planning
- Identify opportunities to integrate Advance Care Planning into practice
- Increase confi dence in having an Advance Care Planning conversation
- Learn techniques and strategies from one another around
- Consider when is not appropriate to have such conversations
