As a premier nonprofit organization dedicated to providing compassionate care and support to individuals and families facing advanced illnesses, The Foundation is thrilled to unveil its refreshed identity that better represents its mission and values.
Formerly known as “Community Hospice & Palliative Care Foundation,” The Foundation of Community Hospice & Palliative Care now embodies its commitment to fostering a sense of community, compassion, and empowerment through its new brand.
With a renewed focus on delivering exceptional care, The Foundation aims to positively impact the lives of those in need.
The Foundation’s new website, thefoundationcares.org, has been thoughtfully designed to provide visitors an informative and user-friendly experience. It serves as a comprehensive resource, offering valuable insights into The Foundation’s programs, services, and impact stories.
In addition to their new website, The Foundation has also created their own social media channels, Facebook, and Instagram, to engage with donors and supporters.
As part of the launch of those channels, The Foundation is asking supporters to post pictures of ways they support the organization, from volunteering through their Teen Advisory Board to simply offering financial assistance for the care Community Hospice & Palliative Care provided their loved one through their #IAmTheFoundation.
The Foundation raises funds to support the mission of Community Hospice & Palliative Care, a nationally recognized, locally based non-proﬁt, committed to compassionately caring for the needs of children and adults facing life-limiting and advanced illness.
With a focus on enhancing the quality of life for patients, Community Hospice & Palliative Care offers a wide range of programs and services to address the physical, emotional, and spiritual needs of those in their care.
