The first discussion document is a synthesis of previous Parliamentary and Senate reports that summarizes the recommendations from the relevant national reports on hospice palliative care over the last two decades. It has been made available online by clicking here. The second discussion document describes the palliative approach and considerations for pan-Canadian implementation and how the palliative approach can address care for people with chronic and life-limiting illnesses other than cancer. The document considers all settings of care including primary, acute, long-term, home and hospice and addresses strategies that have been successful in integrating the palliative approach to non-cancer illnesses across these settings. The chart, “Chronic Disease Continuum Model” will be updated at a later date once translated into French by the Ministry of Health Ontario and revised in a clearer visual resolution.
