At the Marie Curie Hospice, Hampstead we have a new and exciting opportunity for an Admissions and Discharge coordinator to pro-actively co-ordinate our admissions and discharges throughout the hospice, managed by the In Patient Ward Manager and working closely with the Multi-Disciplinary Team.
The hospice is a busy unit and our aim is to care and support more patients and their families, so you will be responsible for ensuring effective patient flow to enable patients access to our care when they most need it.
You will be the first point of contact for hospice referrals and able to carry out initial clinical triage, so it is a requirement of the role that you are NMC registered with hands on clinical experience at a minimum band 5, ideally within a palliative care setting.
If you enjoy working at a fast pace and have a problem solving approach you may be ready for the opportunity to develop your organizational knowledge and skills. You will be part of the Senior Nursing Team and will work alongside the MDT to ensure that the hospice capacity is managed effectively.
The role will also involve fostering close relationships with key referrers and promoting the hospice to new referrers, so that we can widen our reach to more patients with a terminal illness.
Whilst we provide 24 hour care, the hours of this role are predominantly Monday to Friday, though there may be occasion to work outside these hours.
