The awards are now open for entries and aim to celebrate and recognise organisations and individuals who are at the forefront of offering integrated care that removes the barriers between primary, community, secondary and tertiary care and between health and social care.
There are 12 categories, including:
- End of life care
- Pain management
- Dementia care
- Cancer care.

Previous winners
This is the first year there has been a specific ‘end of life care’ category, but last year a couple of end of life care initiatives won awards in other categories.
The Blackpool Teaching Hospitals NHS Foundation Trust won the award for ‘Cancer care’ for its rapid discharge pathway for patients at the end of life. The pathway helps facilitate a safe, smooth and seamless transition of care from hospital to community for patients at the end of life who wish to die at home.
The Cornwall and Isles of Scilly Primary Care Trust and Peninsula Deanery won the award for ‘Dementia care’ for improving end of life care decision making for people with dementia. A new model involves families of people with dementia in ‘best interest’ meetings, where future care is decided on and planned.
