CLINICAL NURSE SPECIALIST (2 Permanent Posts)

Salary: Band 7 £37,659 – £47,778 per annum inclusive

Hours of work: 37.5 per week

We have a range of exciting opportunities within the Community Specialist Palliative Care service which includes the First Contact team. The First Contact Team is a dynamic “one stop” service, acting as the 1st point of contact to all St Joseph’s services including initial holistic telephone assessment and triage. The service also provides rapid response home visiting within 24 hours, to those patients triaged as urgent. The Community Teams work across three boroughs; City and Hackney, Newham and Tower Hamlets. We work closely with our colleagues in community and primary care to help deliver integrated and partnership working, through joint working, education and training.

We are looking for enthusiastic and highly motivated nurses to join our team. These exciting roles offer the opportunity for dynamic and self-motivated nurses to utilise and further develop skills within a dedicated palliative care role.

The teams provide a responsive service, providing specialist palliative care advice including the ability to deliver safe evidenced based symptom control advice, alongside the provision of social, emotional and spiritual care for the patient and family. You will work closely with, and act as, a resource to primary care teams and other care providers with a common aim of achieving the best quality of life possible for our patients.  The post involves working on a 7-day week rota.

What we are looking for:

Post registration experience in palliative care or related speciality, highly developed communication skills, the ability to practice autonomously and experience of working in the community. Experience of teaching, research and audit and ideally have, post registration study in palliative care or be prepared to undertake this.

The Team would be willing to offer developmental CNS post to candidates from clinical back ground out-side palliative care who meet the majority of the person specification
