Community Specialist Palliative Care Social Worker (22.5 hours p/w)

Based in the Barnet Community Team
Grade: NJC Scale 38-39 – (£34,936 – £36,012 (pro rata))
(Newly qualified candidates with palliative care experience may be considered at the appropriate grade)

We are looking to fill a key post in our Social Work Team.

The Team is part of Supportive Care services, which also include volunteering and spiritual care. The Specialist Palliative Care Social Worker will assess the most complex need alongside a Loss & Transition Specialist overseeing volunteers pre- and post-bereavement to ensure sustained support.

You will have a qualification and substantial experience in Social Work, preferably a familiarity with palliative care. You will be confident in working with emotional and relational complexity and risk in a multi-disciplinary context. You will lead creatively in guiding other professionals and volunteers in the assessment, planning and provision of specialist palliative care.
