In a new and exciting initiative Overgate hospice are looking to appoint an exceptional individual to deliver education, training and expert support across Calderdale in Specialist palliative Care and End of Life care . This is part of a joint partnership with Calderdale and Huddersfield NHS Foundation Trust and Marie Curie and Overgate Hospice to improve local end of life care services across Calderdale. We have recently been commissioned to provide and deliver a programme of end of life care education for clinical teams within the Calderdale community settings to strengthen end of life care for patients and families. This will include GP surgeries, district nurse teams and care homes.

The post holder will be responsible for:

  1. Providing a local leadership role with respect to education and training in end of life care tools ensuring end of life training and education is embedded across the locality.
  2. Strengthening the whole system approach to end of life care
  3. Close working with the EOLC care training project
  4. Education to improve primary care professionals’ identification of patients at end of life and supporting primary care and specialist palliative care professionals in developing an end of life care register 
  5. Education to improve consistency within and between clinical teams, highlighting and feeding back to the project team areas for improvement.
  6. Improving working between organisational boundaries
  7. Contribute to end of life care education, sustainability and developments
  8. Work closely with the specialist palliative care teams in hospital, community and the hospice.
  9. Work collaboratively with the Calderdale and Kirklees Palliative Care Education Group (Chair – Dr J Ackroyd, deputy Dr R Sheils
  10. Work collaboratively with the newly formed west Yorkshire sub regional end of life care facilitators group

We will need to evaluate the impact of the nursing and educational roles to help inform future service needs.

We are looking for an enthusiastic person with desire and drive to improve end of life care for patients and their family – You will be:

  • A Registered Nurse
  • Recent extensive experience in end of Life/ palliative care nursing 
  • Experience of delivering education and training in the work place setting 
  • Have a post registration Palliative care qualification 
  • Have significant experience of facilitating learning in clinical practice including formal teaching
