End of life care project facilitator

Vacancy/secondment opportunity for a 12 month period to work within Western Cheshire (This post may have an opportunity for extension to 2 years)

You will facilitate and support the implementation and evaluation of specific projects and initiatives around palliative and end of life care across all care settings with Western Cheshire – acute care, community and primary care, care homes, hospice, social care and the voluntary sector. 

You will support the rollout of initiatives such as facilitating education, working with palliative care champions groups, gathering evidence and furthering best practice in end of life care priorities such as the use of the Supportive Care Register, coordination of information and advance care planning.

You will also be required to support the development of robust partnerships across organisational and professional boundaries, working with key health, social care and third sector personnel, as well as liaising across care settings in order to promote and develop communication and continuity of service developments.

You should be dynamic and resourceful with excellent interpersonal skills. Committed to supporting consistent, measurable improvements in end of life care for patients and carers, you will have a sound knowledge of palliative and end of life care across different care settings, and the skills to develop robust partnerships. You should be enthusiastic and energetic, customer focused with a mature and flexible approach. You will have excellent interpersonal and communication skills, along with negotiating and influencing skills; you will be able to work to tight deadlines and have experience in prioritising multiple tasks.

Salary: Band 6, £25,528-£30,460 per annum (dependent on experience)

Hours: 30 hours per week
