Salary: £39,000 – £43,000.00 per annum

St Catherine’s Hospice provides inpatient, day care and community specialist palliative and supportive care to people with life limiting illnesses and their families within Sussex and Surrey.
We are seeking an innovative and motivated individual, who ideally has specialist palliative care and case management experience, to join our team as Lead Practitioner for our new Palliative Care Co-ordination Centre. This service will provide a single point of contact and access for all new referrals to the hospice, as well as providing round the clock advice and support for patients, families and health and social care professionals.
You should have sound leadership and management experience, as well as excellent communication and interpersonal skills. You should also be able to work using your own initiative, have excellent prioritisation and assessment skills, be able to give clear instruction and lead and manage a team of practitioners and administrators.
