Salary: Band 5 £26,030 – £33,816 per annum

Hours of work: 37.5 per week

An excellent opportunity exists to join our Live Well Information and Support service, and provide outreach in City & Hackney, Tower Hamlets and Newham. The service is based at St Joseph’s Hospice in Hackney in partnership with Macmillan Cancer Support.  You will work in a pioneering community based service which offers drop in, peer support and self-management opportunities for people affected by a life-limiting illness.

The successful applicant will have experience of working with and alongside people living with a life-limiting illness, providing information and support services. You will have highly developed communication and IT skills, be committed to service innovation and want to be part of a team establishing an outreach service. You will be required to develop local partnerships, working in our diverse communities to ensure that our services meet every person’s needs. We would particularly welcome applications from people who have a strong connection with our local East London communities.

The job will be based at St Joseph’s Hospice in Hackney in East London. Travel in and around the local boroughs will be essential.  Full support and training will be given in all aspects of the job by the hospice and Macmillan Cancer Support.  The post holder will be directly accountable to the Live Well Information and Support Team Manager.
