Manager of Patient and Family Support Services

Salary: £35K – £37K p.a

Hours: 37.5 hours per week

St Clare Hospice is a leading provider of palliative care in West Essex and East Herts borders.

We are looking for an enthusiastic and motivated Senior Social Worker to lead and manage a small team that delivers assessment, care planning, bereavement, and psychological support and facilitates safe discharges from St Clare Hospice, for the community of West Essex and the border of East Hertfordshire.

The post holder will be experienced in service development and lead on the development of our bereavement services; our strategy is to further expand our current provision and set up a children’s bereavement service. Therefore we are looking for someone with significant experience, with innovative ideas to provide a creative approach to service delivery with the ability to effectively manage change.

You will be a qualified and highly experienced Social Worker with excellent people management skills and healthcare experience, preferably with a bereavement qualification. You will have an inclusive and dynamic approach, preferably with community engagement skills.

Experience of working in a hospice, palliative care setting or joint health and social care environment would be advantage. A flexible attitude to work and the ability to adapt to changing patient need is important.
