This is an exciting opportunity for a Social Worker to join our team as we develop a new and innovative service called ‘Gateway’. The service will manage patient and carer/family access to all St Richard’s Hospice Palliative and End of Life care services and Bereavement Support South Worcestershire.

The post holder will be part of the service from its introduction. Phase 1 of the project will launch this autumn and will initially run Monday to Friday 8.30am – 5.30pm but in the longer term will run 7 days per week from 8.00am-6.00 p.m.

The post holder will be part of a multi-disciplinary team contributing to managing all external referrals and will:

  • Provide support and advice by telephone, assessing patients, carers and families (including children) to establish the most appropriate support and care and referring on appropriately
  • Provide a crisis intervention response if required
  • Advise and participate in multi-disciplinary assessment and care planning and liaise with statutory services as appropriate

Eligible applicants will be currently registered with the Health and Care Professions Council with ASYE or equivalent and at least 3 years’ experience post qualification. Experience of working with people living with life threatening illness and those who are bereaved are also essential requirements of this role as is the ability to work flexibly and innovatively.
