Salary: £30,227-£36,124 per annum, pro rata

We are looking to recruit an enthusiastic, experienced and motivated Senior Practitioner within the Specialist Palliative Care Social Work Department.
The post holder will be responsible for an efficient, effective high quality service for all service users referred to the team. You will liaise and work with the MDT and colleagues from the statutory and voluntary sector to formulate clear plans for service users and in particular give priority to complex circumstances/situations. You will also be able to demonstrate proven clinical leadership and have contributed to the improvement of innovative change to clinical practice within the specialty.
A qualification in Social Work plus current membership of Professional/Registration bodies is essential. Significant post qualifying Social Work experience, incorporating the need to demonstrate evidence of senior responsibility within the workplace is required. In addition, evidence of partnership working including experience of involvement with Continuing Health Care/Local Authority is also required. Awareness of Specialist Palliative Care Social Work issues is essential along with experience of working within a Multi-Disciplinary Team.
The successful candidate must have experience of supporting people through loss along with being an advocate on behalf of service users. You must have experience of staff support/appraisals and have the ability to negotiate involvement across all levels of professionals both inside and outside the organisation.
