SERVICE DISCHARGE COORDINATOR

SALARY RANGE £29,788 to £31,883 PA, PRO-RATA
30 HOURS PER WEEK -18 MONTHS FIXED TERM CONTRACT

Are you an experienced clinician with excellent leadership and strategic planning skills and a passion for developing innovative services?

A new and exciting opportunity has arisen for an experienced Health and Social Care worker to clinician to join the Saint Francis Hospice team as the Service Discharge Coordinator to manage the referral and discharge process for the 18 bedded Palliative Care Inpatient Unit.

Saint Francis Hospice is a dynamic, forward thinking organisation delivering a full range of specialist palliative care services for all patients. Clinical services are provided within a modern, personal and friendly Inpatient Unit, in the Community we have the Community and Crisis Support Service and Hospice at Home Service as also provide Specialist Day Therapy Services. Alongside these clinical services we have the hospice Pepperell Education Centre which has an excellent reputation for delivering high quality education programmes and study days.

The postholder will be joining Saint Francis Hospice at an exciting time of service reconfiguration and change as services are adapted to meet the changing needs of our local community. The Inpatient has recently undergone new leadership reviewing reporting structures and staff establishment to support our patients increasing complexity and needs. This has opened up an opportunity for a pilot role for 18 months for a Service Discharge coordinator. The focus of this role will be to work as a member of the MDT team, providing high standards of specialist palliative care to individuals and their relatives, providing a role model to other members of the team. Lead the daily admissions and referrals bed meetings, manage the allocation of new patients to current bed status/skill mix in close consultation with IPU senior nursing and medical teams. Ensuring the smooth pathway from admission to discharge by developing and maintain systems and pathways for all discharges from the hospice, working with the whole multi-disciplinary team.

If you are a highly motivated and dynamic clinician with strong administrative and operational leadership skills and can adapt and manage the change process we would love to hear from you. Excellent team working skills and strong interpersonal and communication skills are essential for this post to support sensitive collaborative working with colleagues, volunteers and patients.
