Service Improvement Project Manager

The charity Sue Ryder is working in partnership with the company Dynamic Health Systems (DHS) to pilot an extension of DHS existing internet delivered “VitruCare” supported self-care service to offer help to people who are approaching the end of life initially in three pilot areas: Leeds, Keighley in West Yorkshire and Bedfordshire

An experienced service improvement manager (home based) who can work across organisations involved in supporting patients and carers at the end of life. The successful person will be piloting new ways of coordinating an individual’s care, ensuring their wishes and choices are always known to those supporting them and of obtaining relevant information regarding both their condition and locally available services. There is a requirement for significant travel (paid from work base) between the pilot areas and occasional further afield therefore the post holder must be able to work flexibly.
