Transition Support Worker

Salary: £23,500 pa.

We are looking for an enthusiastic and organised team player to join Richard House as the Transition Support Worker, a unique role in partnership between children and adult hospices.

You will have a minimum qualification in youth or community NVQ level III and relevant experience. We welcome applications from graduates in health, education or social care with the relevant experience.

The role will entail developing and maintaining effective partnership working with families with a commitment to promoting the welfare of children and young adults when accessing services at Richard House and whilst delivering community outreach visits, day activities and group sessions.

To work with families, children, young adults and other agencies to assess, provide and co-ordinate interventions to meet identified support needs to achieve seamless transition from children to adult palliative care.

To facilitate and deliver support and interventions which improve parental capacity and empower children and young people to achieve the best outcomes for their life-threatening and life-limiting conditions and disabilities, in partnership with internal and external clinicians and partners

To participate in the wider children and adult hospice services with a particular emphasis on targeted vulnerable children and young adults to engage actively with the services.
