This is the inaugural meeting of the APM’s Transition Special Interest Forum.
The forum aims to improve services for young adults with palliative care needs and to increase awareness amongst adult specialist palliative care professionals of the needs of these young people.
The meeting will provide an opportunity to network and enable sharing of ideas, developments and best practice.
